In July 2005, A. Kevin Raymond, MD, was a guest speaker at the orientation of the new pathology residents at The Methodist Hospital, Houston, Tex. The topic of the lecture was "The Paragraph System," a gross pathology dictation system that he had developed and successfully used at his own institution, the M. D. Anderson Cancer Center in Houston, Tex. The rationale behind this system is that gross dictation training is inconsistent; it is not taught, not taught well, or is lacking the follow-up that truly enhances the learning process. From our own experience with pathology residents, fellows, and pathologists' assistant students, traditionally organized gross dictations often result in reports in which pertinent information is buried within long meandering paragraphs.
